Josephine Ortiz of the San Antonio Food Bank Benefits Assistance Team will be on-site to help you through the process of screening, applying or renewing your benefits for SNAP, CHIP, WIC, Medicaid and more! Documents to bring (if applicable):
- Social Security Cards, ID or birth certificates for all household members
- Proof of Income: Last 4 paystubs, Social Security Award Letter, etc.
- Rent receipt, lease agreement or mortgage payment
- Most recent bills for telephone, electricity, gas, water, insurance premium, medical bills and prescriptions
- Current bank statements, life insurance policies, tax statements for real estate, etc.
- Immunization records: TANF and children's Medicaid only
